FIRST + NEXT STEPS

  1. Set Up an Appointment. The first step to setting up an appointment is to call 563.556.0699 between the hours of 8:30 am - 1 pm, Monday through Thursday.
  2. Navigate the Insurance. Lori* will help you navigate any insurance questions you may have. [Private pay is also an option.] The first time you meet with a counselor we don’t want you to have extra stress in regards to payment. Our goal is to have the paperwork completed and payment squared away so all energies are focused on your main concerns.
  3. Understand the Billing Process. Bernie* takes care of the billing. If there are any questions or concerns that arise down the road, she will be happy to help.

*both Lori and Bernie have been at Crossroads for more than 10 years and are quite helpful and knowledgeable. You're in great hands!


Major Insurances. The major insurances we take are Wellmark, United Healthcare, and Iowa Total Care. Contracts change,

so it's important to check with your insurance company to see if we are a provider.
